Laparoscopic Management of Benign Metastasizing Leiomyoma in Deep Retroperitoneal Space: A Case Report and Review of the Literature

Abstract
Benign metastasizing leiomyoma is characterized clinically by metastatic features, but histologically by benign smooth muscle tumor. We present the case of a patient with a benign metastasizing leiomyoma incidentally found in the retroperitoneum. A 56-year old woman underwent an abdominal hysterectomy due to a uterine leiomyoma 23 years previously. The patient was referred for the evaluation of an asymptomatic solid mass detected in the pelvic cavity. Clinical examination revealed no evidence of malignancy; the serum tumor markers and the systemic work-up were negative. Abdominal-pelvic magnetic resonance imaging was performed and revealed a 5 cm mass with a solid component in the left pelvis. Observation under laparoscopic guidance showed that the uterus was absent, the ovaries were reduced in size, bilaterally, but grossly normal. A protruding mass from the left retroperitoneum was noted. We opened the retroperitoneum and removed the mass by enucleation and subsequent morcellation. The histopathology confirmed a leiomyoma with no mitosis or nuclear atypia.
